Please complete the form below and allow at least 24 hours (Monday-Friday, excluding weekends) for processing after submission. This allows our veterinarian to review your pet’s records and confirm if the requested medication can be dispensed.

Important Information:

• Your pet must be a current patient of the clinic and must have had a veterinary examination within the last 6 months.
• Some medications may require a recent consultation before they can be dispensed. This decision is at the discretion of the attending veterinarian.
